Unfairly treated and poor communication

On 01/07/2013 I handed my iPhone 4s in for a damage report. I have a contract with MTN for the iphone 4s that is approximately 1 and a half years old. The LCD screen had stopped working. The responsible person took my phone, and gave me my sim card, but not the sim reader that goes with it. More than 2 weeks later, they were unable to tell me why I couldn't get a damage report for my phone, but simply that it had been sent back to the shop. On 22/07/2013 I went to the shop in order to retrieve the phone and request the damage report. No damage report was found, and they said they could not provide a damage report due to the sim reader not being available. I did not take the sim reader, which means that they have lost it. It has also taken 22 days and multiple phone calls, none of which were returned, to come to this. I now sit with a broken iphone with no sim reader and other companies cannot assist with the damage report (which I need - my phone is insured with my insurance company) because it is not an MTN phone.

I am unimpressed because of:
- Lack of communication
- MTN lost my sim reader
- Now refusing to provide a damage report on my phone
