Card not received up to this day

I joined the scheme in January 2013, up to this day i have not received any welcome pack and my medical aid card. I have been patient and really this is just not the service I will settle for. All i want is my card and the welcome pack. We are in april already and no card has been sent to me in any way wathsoever. No constant follow ups have been done to confirm if I got the card or not. I keep phoning Remedy for my card from my phone! PATHETIC SERVICE
