Being Charged for insurance, no permission granted

I received an email on Tuesday 30 April 2013, informing me that my insurance on my device is effective as well as the details and conditions of the insurance. Now I have two main issues.

1) I never requested insurance coverage for the device and,
2) The details on the attachements have incorrect details.

I have been trying to contact the call centre number (086 1000 804) provided in the email. I have contacted this number since Tuesday and to date have just heard "your call is important to us, please stay on the line"

I am deeply concerened that money (irrespective of the amount) can be taken from my account by someone to whom I have not provided permission to.
