Asked for insurance when taking contract

Good day

please inform me why i have not got insurance on my galaxy camera, despite me filling in the forms and sending it with the application for the device. i was told by the sales lady in the Menlyn store insurance will be added onto my account. i had a look at my bank statement and no insurance premiums have come off my account!!! if this device was would you have replaced it??? i want the insurance put on NOW!!!
