Not adding insurances that was requested

I renewed my contract on the 30.04.13 at the mtn store in town and I requested for insurance to be added to my account now that my handset was they say I don't have insurance. I was under the impression the page I signed was for my insurance now after the fact they wrote no insurance on the page that was explained to me I was paying my contact on time every month with expecting insurance to be added now I must pay for a handset that I don't have they cant just write no insurance after I signed the paper work with out informing me I want this sorted out and corrected
