How to wait 7 months to stop a SIM card!!

I took out upgrade on my current Vodacom contract. For some mysterious reason a new SIM card (07972553876) was also issued to me. this number does not work and I received no SIM card.
I was requested to send an affidavit and bank statement confirming that I paid for the connection fees. This was done on 21 January 2013 and again on 12 July 2013.
Reference number S3-SEA3Q-Q9057 : IR031/2013
Nothing has been done!! I have been paying R159.00 per month for a service that I do not use.
