My lost insured phone not replaced

I applied for a contract phone at mtn shop at kempton park branch on jully 2012. they called me after 4 days to come and collect my phone. When i get there they convince me to insure my phone because is more expensive. My phone was Nokia N9 and i agreed for their insurance, they filled the form and let me sign at the bottom of the page and told me the handset is insured right away. I used the handset with the simcard i got from them until it get lost on the 24 of may 2013.I went to kempton park police station to open a case and followed all the procedures needed for the insurence to process my claim. After 3 days of waiting i got a call and email from 24 hour mobility telling me that my claim has been decline, reason was i was using the wrong simcard when the phone get lost, i was supprised and went to the mtn store to find out what is happening. When i get there they opened my file and told me that the number on the form is the one i am using, meaning when they fill the form they only put the prepaid number on the forms for which reason i dont know, While i have been using the phone with the contract simcard in it.they (mtn consultant) ask me to write motivation letter explain to
