TRYING TO CLAIM PHONE FROM MTNSP INSURANCE

I had my phone on the 1st of June 2013, I have been calling 083 123 6084 since then, and have walked into MTN stores. everytime I try doing this over the call centre, keep on getting cut off.
When I walk into the MTN stores, they tell me that they cannot find any trace of the phone on there system. In which case I would like to know what is it exactly that I have been paying for all this time. The worst was when i had an accident on Monday when I had an accident, I the tried to call my husband, only to find that my services had been suspened... talk about stress.
All i want is to put in a claim for a new phone, can somebody please enlighten me as to how exactly doI about doing this, because clearly contacting the call centre (011 123 6084) is absolutely futile.
