Vodacom Debiting My Account Twice In One Month

Herewith I wish to to notify your company that I have requested to cancel the debit order on my account with the following number 0824333123.
I have requested the aforementioned about a month ago and still they are debiting my account.
Now the debit order went off on the 2nd June 2013 & again the amount of R 1934.52 on the 7th June 2013.
How in the can you guys debit my account twice in one month!!! I am not very happy about this as urge you that this debit order be removed from my account as we requested that we will do cash payments on a monthly basis.
I am a pensioner and my monies is being transferred into my account on a irregular basis hence the request to do a cash payment monthly.
Second I have logged issues with signal in our area & to date nothing has been done with this matter!!!
Should my request not be honored I will bring the CPA into this matter & will cancel my contract with your company!!!
I am going to the bank tomorrow to reverse the debit order & I urge you to give me feedback by close of business Monday 10 June 2013.
