No response from "Speak to our MD"

I have been trying to resolve an issue with my membership since Jan 2013 and haven't had any success with the sales agent at Morningside (Mathew), the call centre or with "Speak to our MD".

From 1 Jan 2013 I stopped being a member of Vitality. I went into the gym on 2 Jan to find out what the alternative membership options were. Mathew told me that as a CAMAF member I qualified for a 15% discount and that I needed to pay a joining fee of R150, even though I've been a member for many years. He also said that I would only pay the joining fee in the first month and not the monthly subscription.

Since Jan I've been charged the full R625 pm for national membership. Apparently my membership status says that I'm an ex-Sanlam Reality member. I HAVE NEVER BEEN A MEMBER OF SANLAM REALITY. I believe Mathew incorrectly tried to load my membership with Sanlam, which was subsequently declined, hence my paying R625 pm.

What I've been trying to get clarity on is:
1. Is there currently a fee discount for CAMAF members that aren't with Sanlam Reality?
2. Was it correct to charge a joining fee of R150 in my case?
3. Should I have paid the monthly fee in the month I payed the joining fee?
