Unfair billing

I cancelled my cellphone insurance on the 12/06/2013 at a markham store at westcoast mall. I am still being billed for that same insurance.
The staff at markham tell me that I have to settle my account first before they can close it. An amount I wasn't suppose to pay in the 1st place..

I seriously don't know what to do anymore.
