Application lost in store, and bank cost me money

On the 2013/02/03 at 01:01:50 PM I applied for Internet bundle contract. The consultant Shadene at the store, told me that in about two days she will call me concerning status of my application. But she never did. Two weeks later, I went back and spoke to Sacle Alexander who printed a prospect detail report (prospect ID 1585344) which states reason of termination been Pending information not received.

Store manager Bafana promised that he will look for my form and resubmit it, after he showed me that Head office have made checking on bank, ID etc all is good. Today, a week later. I called him and he said that he lost my phone number and that he can't find those papers and I must re-apply.

It costs me to retrieve bank statement. I need MTN to pay me for bank cost. My ID copy, salary slip and bank statement are lost in store. Now is my phone number. Furthermore, I have to spend time and money get another bank statement which could also be lost.

Bafana said that, the store personnel did submit paper work head office need it. Hence, my application was terminated.

Accountability, MTN you have all my details in your system why do I have go through this?
