Fraudulate upgrade on my contract

I am very disappointed at the lack of service and honesty at vodacom and incredile connection, I want to the voda shop in cresta to pay my bill in October the consultant told me i was due for an upgrade which I opted to do at a later stage, at the end of November i went in to the same shop to do an upgrade on my cell phone contract only to find out that a upgrade had already been done on my cell phone contract, which was apparently done in incredible connection in sandton. i have be sent from pillar to post by not only the call centre but by the forensic depart, I faxed an affidavit state that this was a upgrade on the 27th of November last year but I didn't' receive any feed back. I want to the vodacom shop in northgate early January this year, the consultant phoned incredible connections and asked for the documents that the had signed for the upgrade to be processed, incredible delayed faxing the documents until a week later however the consultant in northgate finally got a hold of the documents and re- faxed my documents to the head office, it is now February and i still have not had any feed back from vodacom.
