Migration business contract under false pretenses

I was called by Elite Mobile in May to inform me that my current contract no longer existed. A smart large contract matched my current usage patterns under my existing contract. I told the telemarketer that I would only upgrade if each and every call cost was shown on itemized billing invoice irrespective of whether it was part of the free minutes or not. So that I could claim the cost of my business calls from my employer who required to see the actual cost of the calls made.
After consultation with their superior (to which I was not privy to) the telemarketer assured me all calls would be shown on the itemized billing and their cost.
The first invoice subsequent to the upgrade did NOT in fact show the cost of all calls but indicated them all as a zero cost. Now I cannot reclaim any of my business calls from my company, cost to me is R150-200/month.
Also when the contract was sent to me it had an insurance policy added to the contract that I did not request at all. I phoned Elite Mobile back to cancel the insurance contract on the phone. The cancellation has still not been successfully processed, i have paid 3 months of premiums with no end in sight. cost to me R85 /month.
