Completely disgraceful service i received

I am writing this report on a very aggravated mood as I have been having constant issues with my medical aid. In December '12 I added 2 people to my plan whom you can add & remove at any given time. In December Bank-med's plans department decided to put me on the incorrect plan which I had to go through to have it sorted as I only realized this when they debited my a/c with R3000 -, I queried this & it was sorted. Last month the same thing happened after the plan was changed to the correct one I wanted but the debits were RIDICULOUS so to speak. I inquired with Bank-med who advised me that my employer (FNB) had been short paying them for my contributions, thus they reconciled & collected what's due to them, WITHOUT notifying me if I may add. This is both a fault on FNB & bank-med as I constantly have to raise disputes regarding my salary in order to get feedback or an explanation that I also have to give a window period of 5 days for response.
I then sent them documents to have my dependants removed from my plan so I can be the only one on it, which you can do at any given time, but to NO they still want to debit me as per their 30 day window rule that i never sign for. HELP!
