My Account debited twice with Std Bank funral plan

I Nkosana Alfred Nkutha identity number 7909255929080 hereby concern about the Standard Bank funeral Plan. On the 26/07/2013 my account was debited with rhe amount of R71 which is my monthly premium.but I knew that In June there was insufficiant funds in my account so that means that debit order was for June and for July I didnt pay as well and I knew that I am still owing one premium of R71 for July. Yesterday when UI get R380 refund from Attorneys I realized that Standard Bank funeral Plan debited me X2 R71 and when I query why they deducted double premium they told me that I have to fax them Bank statement which I did . when I phone them again today, they told me that they havent received statement yet, because even the first time they provide with me the incorrect fax number . so that I went to the bank personally to confirm with the bank .they told me that It is their mistake I have to contact them . even if I contact funeral department I am still not getting the correct responce . please advice
