Claims submitted that is not being paid

I am really getting fed up with this my claims has been submitted exactly the same every time now all of a sudden it is not right!!! how can you pay claims that is submitted every time the same and then all of a sudden it is rejected but the claim looks exactly the same as the previous ones!!! So if the claims get submitted to late it wont be paid as well but no confirmation is given regarding this if I don't phone i will not know!!
