Change of account type without permission

Having monthly fees on a standard Silver Chequeing account, we approached the ABSA branch on the corner of Bell&Frere Rd, EL to change the account to a Silver Value Bundle at R79pm. We approached the reception desk and the lady behind took down the account number on a post-it and a copy of ID and said all would be well. When asked if there was some thing to sign, or receipt she said that nothing was required from our end and she would pass everything on to a sales person. We asked to see the sales person, but he was apparently busy. So we left, a little apprehensive, but with verbal assurances from the lady behind the counter.
However, on inspection of the latest statement we see that the account was being debited R109pm instead of R79. On visiting another branch (25min que BTW) we were told that the account was upgraded to a Gold Value bundle. This was without any permission given nor asked for, nor notification, and apparently could not be reversed without closing the account (???). The original worn Silver Cheque card continues to be used. This is unacceptable and the account needs to be rectified to a Silver Value bundle as requested and the excess charges refunded.
