My RA (Policy number 002 550 4754) matured in Feb 2013. I received notification from Liberty on the 17 December 2012 that my policy would mature and I should make a decision about what should happen with the proceeds. I completed all the paperwork and forwarded it to Liberty early in February for transfer according to my preferences by end of February. You guessed it!!! NOTHING has been done. Don't get responses to my e-mails, no contact numbers available to follow up.
