Wrong credit information supplied & Poor response

I've got a problem with Standard Bank credit card division (Debt Review Services), I was under Debt review which I also wasn't helped properly and paid for no services rendered. I only found that out when the company closed and then had my name removed from Debt Review last year June by the NCR. I then checked by all the Credit Buro's and my name was cleared. My wife made arrangements at the bank for outstanding amount owed after I payed a lumpsum. I went to my bank which i had a loan at and a multi-loan, they told me they can't help on the ML anymore because Standard Bank put me up on for Debt review. I "ve been sending emails since December 2012 and letters showing that I'm not on Debt Review anymore, with very slow response, I got a letter last week now saying that my wife is also under Debt Review. I contacted the NCR and according to them her name are not on for anything... i did sent them the reply from NCR and asked them to sort this problem out ASAP. I am still waiting on a response and clearance on this matter, this is causing a bad name for me and reflects bad on my credit score. This is really unprofessional.
