Request for registration form

Please assist, I settled my account on the 7th October 2013 and still waiting for my registration forms via courier as requested. I sent plenty of emails to care@mfc and I kept receiving an email stating that my query have been forwarded to a relevant department and will revert back to me shortly. Alternatively I called the callcentre and cannot reach the consultant, No answer. Its been too long now and I m wondering if I will ever receive them. Please assist me I really need those registration forms asap
