No explanation - my declined query

It is with great disappointment that during last year September 2012 there has been some discrepancy going on with my account and no one has bothered to investigate this matter. Money has been deducted from account without my awareness. I have lodged a complained, then my card was blocked and I was given another card to use. A month later in November 2012 again money was deducted from my card, I went back to report this then a new card was given to me. On 3 January 2013 I went to the bank to make a loan, then on the same day after receiving the sms notification that my loan (R15, 000) was successful an amount of R500 was withdrawn. On the 5/01/2013 when I went to the bank to withdraw R3000, but had reacched my daily limit. The slip showed R500 was withdrawn 2 days before. Then again R500 was drawn then same day I went to the bank. Then when I went to the bank for investigation, they told me that money has been withdrawn from my account I have to change the card again. Since January no fruitful feedback has been given to me. I want explanation and refund into my account together with statement that states what transpired. Why would they give me a duplicard thta is a duplicate of some1
