Blocking of incorrect cellphone

My x and I broke up, I have a top up Contract with Vodacom but he doesnt want to return the cellphone that is on contract. I blocked the sim card but wanted to block the cellphone as well as he doesnt want to return it to me and says that if he has to return it he will bring it back to me in pieces. I tried to see that if they can block the CELLPHONE without the IMEI nr because of him having the cellphone box and the cellphone. Vodacom then said (call centre agent) that they can retrieve the IMEI number without having the phone or28 box. I went to vodacom at East Rand Mall where I asked for assistance, a lady said that the IMEI nr that was listed wth the blocked cellphone nr was written on the page that was sent in, later on thursday my phone was blocked instead of my x's phone, now because of THEIR error I have to go to a police station and get an a form and un blacklist it, they said I have 3 IMEI nr's on my name, but I only have 2 Cellphones on my current contract! How can this be?? Why should I go to the police to get it sorted when surely it was an error on their side, I am seriously upset and got into so much problems at work with regards to driving up and down???
