STILL waiting for a return phone call and feedback

At the beginning of February I dropped off 'change of business name' forms & all back up documentation at this Vodacom shop. I was told they'd be handed in to the correct person to do this (a lady who i since hear no longer works there).
At the end of Feb i phoned to follow up and no one knew what i was talking about - there are no signs of my forms there and the company name is still reflecting in the old name.
I was told by the manager (Morne I think) that he'd phone me back by close of business - that was 3 weeks ago. When i phoned 2 days later i spoke to Jayson and he'd also phone me back with feedback... still waiting. I phoned last week - didnt get the guys name - he said he'd get someone to call me back... guess what... still waiting.

This is ridiculous - No one can pick up the phone, phone me and let me know whats happening. Can someone possibly go and look for the forms for me?!

More than likely i have to go back to Vodacom, redo the forms and and start from scratch!
