Cell C Early Upgrade

Hi

I have two cell phone contracts and one of them the contract ended on 23rd of Nov13.
I called the call centre asking them to send me cancellation form to cancel the contract before the 23rd.
When I called in to confirm receipt of the cancellation form with Cell C they have told me on their system that I already upgraded this contract and that the iphone5 was delivered in August this year.
I was totally shocked on hearing that startling news and was very disturbed.
I have being repeatedly calling the Cell C upgrade department and Customer Care as well as emailed one of the customer service consultants.
Till now no one has given me feedback or called me regarding this / on my cell phone.
Please can someone from Cell C head office address this matter as urgent as I am concerned about my name being blacklisted and someone has forged my signature.
This is very stressful and frightening cause what else can someone do with my identity.
I have been with Cell C for a number of years and I think this is not acceptable.
It makes me feel that maybe this is an inside job where as Cell C could be involved.
