Wrong Sim card and Wrong Contract

I took out a My gig 3 from Game in Richardsbay. The consultant then filled in the contract half, his name does not even appear on it.
Sim card that was supplied was incorrect - it should have been a micro sim
Consultant never opened the tablet or inserted the sim card (this i was advised by a vodacom outlet is the correct procedure and what he should have done.) So if the consultant did his job properly he would have noticed this is the wrong sim card.
Then he also never took down the sim cards number and registered it on the contract.
The Game in Richardsbay kept on phoning me for the details (what is this, incompetence or what)
So upon checking my latest statement i see that as of the 16.10.13 i am being charged for a My Gig 2??? What is this i have a contract signed by me for Game for a My Gig 3? And this was taken out in September and now this contract shows a starting date of 16.10.13.
I would like this whole contract cancelled as i feel betrayed and did not recieve what i asked or signed for and according to NCA i have a right to cancel. as this is changing or amending a contract that has been signed.
