Not getting back to me about my new contract

I have asked a sales agent for my new contract after waiting 3 weeks to get my old one canceled and after a week i still haven't gotten the new forms yet and I am waiting for the forms still so I have decided to move to a new ISP and Mweb Still doesn't want to send me the cancellation forms please can you guys correct this for me
