Contract upgrade

I received an sms from vodacom on my cellphone telling me that my contract has been extended to 15/08/15 and my upgrade to smart s with be activated from the 31/07/13 . i then contacted to enquire about this, a call centre agent said yes a phone sumsung S4 has been issued in my name, I informed her that I did not request the upgrade and no handset was issued to me . she advised that i send a copy of my id and an affidavit, which i did the very next day . two weeks later the issue is is still pending every time i phone in for progress, worst of all an amount of R428, was taken off my bank account by vodacom for a phone i know nothing about! I'm still waiting, in the meantime, I'm paying for their mistake!
