Account opened without my consent

A clever person cloned my details and opened a cellphone contract with Vodacom, i picked up a debit order on my account on the 17th of July 2013 i immediately reversed it at my bank, i then called the call center to complain and request procedure to cancel the contract, i was told to do an avidavit and take it to the nearest Vodashop (Centurion Mall) which i did on the 18th, i was then given an e-mail adress [Email Removed] to send the avidavit and was told that they will call me within two days to sort out this issue. I then decided to go to Customer Centre @ Sandton on the 19th they blocked the cellphone no and i was also promised that i will be contacted to sort it out and to date i haven't received a single call from Vodacom. Upon my investigation it turn out that the account was opened @ Eastgate Chatz Cellular.
can somebody please contact me i need this issue sorted and the perpatrator to stop using my name.
