Billed TWICE for an upgrade that never happened

I upgraded my contract on 0835072069 end of may. the phone was not working so I returned it to campus square in Auckland park jhb the very next day. I even received an sms saying that the upgrade was reversed.
today I receive my bill and to my horror there is the upgrade charges for a phone I do not have but not just that. an upgrade charge of R92.10 excl vat TWICE. one on 1st june and one on 4 june. how can I be charged for something I never did, twice not even once.
I want this rectified as I will be taking this to the papers. this is and it just shows how corrupt mtn really is. If I didn't check my statement I would never even have know what a bunch of you people really are.
if its not faulty units or refurbished handsets that the public must deal with, its now of my money.
and you can check where my sim card has been in too and you will not pick up anything other than a S4, wave or a nokia X2.
