Dont hold me hostage to your services

I am deeply displeased with the services of Vodacom. I pay a premium of R20 000 a year, and expect better.
To date, no one has been willing to assist me, and thus, I regret to inform you, that I will insist that my contract be cancelled.
My phone will not be repaired due to my rejecting a quote to repair the external screen. However, I gave my phone in to be repaired, as a result of the phone constantly rebooting. The one damage does not affect the other. Please don't test my intelligence with this.
I have enquired several times, and have not received a call as of yet.
I reserve my rights, to cancel a contract in the event that I am not receiving service. I do not see why I should continue to pay a premium, for a service that I am not receiving.
I further insist that I am refunded 2 months premium, for data I did not use.
If I will not be contacted in the next hour, I will escalate this matter further, that I be refunded my entire year premium, as well as report the Sandton City branch to the CPA.
In summary, these are my demands
1.) My phone IMEI be repaired and delivered back to Sandton City branch by no later than
