No response

I submitted a subcriber cancellation form on the 4th of March 2013 via fax, to the numbers on the form. When I followed up with them on the 2nd of May, they said they did not receive my forms and I should e-mail them, but there is no e-mail of the form. The consultant gave me an e-mai address and I received an automated response with no reference number, saying that I will receive a call from a consultant shortly. I still have not received any feedback as of today.
