I have completed a form to add my husband as a dependent on my medical aid. I have indicated on the form the starting date. This was in Nov 2012 when I sent through the form. I have asked recently what the hold up is. Momentum sends me another form to complete. I have explained on the mail that you should have a form and that I would prefer that a consultant calls me back. I have not received any feedback. all i get is an automated email to tell me that my email has been received and a reference number. I am not used to this type of service from Momentum, and I promise you I will leave.
