Virgin Active fails to cancel contract & incorrect

I called the Virgin active call centre in January to cancel my membership and they have still continued to debit me on a monthly basis, after returning these debit orders they now want to list me on ITC for a service that they failed to cancel and a service that I have never used!!!
I was initally mislead by the sales consultant that's the reason why i cancelled my policy
