Cell phone insurance

For over a year now I have been charged cell phone insurance and I keep saying I do not want it but I am being forced to pay for it with my monthly statement,

I have been in store and tried to cancel it but they still seem not to. I have emailed numerous times and no response.

I have even written on the forms and receipt of the cell phone I bought I do NOT WANT ANY FORM OF INSURANCE.

Yet they are still include it on my account.
