Account was opened on my name

I need a feedback with regards to an account that was opened using my identity number, I have send an affirdavit in January as per requested by home choice and scanned it through to them, however nothing has been done so far, I demand to know what has happened and my name be cleared off credit bearau urgently, I have been calling for feedback and the last agent told it is still under investigation, this is totally unacceptable, six months, still no feedback, and the agent told me there is no sugnature on the application form and told me an incorrect postal and contact telephone numbers, how dare this has to happen? this means there is no account without signature on the forms and this is clearly an inside job, therefore i need a letter confirming that this matter has been dealt with and apology letter, and i do not appreciate anyone harrasing me about when am i going to pay for an account i do not know about, the only call i am expecting is the feedback call from you guys that's all, or we will meet in court when im suing you.
