NON-PROCESSING OF THE DEATH CLAIM

Good day!
On the 15th of April 2013 I applied for the UIF, death benefit, for Mr BVG Zwakala at the Department of Labour, Carolina Branch. I was given a return date of 17 May 2013, should the claim be not processed. On the 30th of May 2013 I went to Department of Labour, at the Elukwatini Brach because the Carolina officials were based at Elukwatini due to office issues. I was told that the form for the banking details was outstanding. I submitted the form that day and was told to return after 10 to 14 working days. On the 18th of June 2013 I returned to the offices of the Department of Labour, the Ermelo Branch to check the progress but was told that the banking details form was still missing. I re-submitted on that day and was told to return after 10 to 14 working days. On the 22nd of July 2013 I called the Department of Labour, Ermelo Branch, to check on the progress and was told that the same form is still missing and was told that they can't make a follow up on my claim as I didn't do the application with them but the Carolina Branch. The only challenge with the Carolina branch is that they still have the offices issues and can't be found afternoon.

Kindly assist with this.
