10 years to get my card delivered

I am still waiting for my medical aid card. I have been promised more than 5 times now that my cards will be delivered (Vitality and membership). I cannot wait for over 4 months to recieve my card when its something that can be done quickly.. I have been paying for my vitality for months now and I still have not yet recieved it, where are the benefits in that?? please, SHAPE UP!!
