Unable to pay, after authorising

I was admitted in hospital, and before i was admitted the hospital contacted MOMENTUM for authorization, ans they authorized and made me pay admission fees and other fees, a month or two later the hospital phoned me and told me my medical does not want to pay, they asked me to fill a form "Clinical History report" so that they can help me.

I then arranged with my GP so that i can come and Fill the report with him, went to the GP and payed consultation fees just to fill the report, and at the end of the day the still didn't pay, and they never even gave me a reason why they don't want to pay, now am sitting with a hospital account which I have to pay and PAY THE MEDICAL AID AT THE SAME TIME.

I don't what more to do Please advice.
