Money taken from my account

My credit card was settled in September 2012. An amount of R941.12 was taken from my account on the 28/12/2013. The amount was reversed but not in time so my insurance premium was returned. I require a letter of apology as well as the R28 returned debit order fee that I was charged, and reassurance that this will NOT happen again.
