NO BANK STATEMENT RECEIVED IN 9 MONTHS

I've opened my Nedbank account in September 2012 and to date never received one bank statement, despite numerous phone calls to the bank and even a visit to the Ruimsig branch! There's also a debit order on the account which goes off twice a month for the exact same amount to Nedbank life insurance (surely I am one person who has to pay one premium?) I also asked numerous times for them to take that debit order off my account as I only have to pay ONE premium! Every month they phone me and tell me the account has been overdrawn with the amount of the second debit order.
