Total inconvenience!!!

I got married in nov 20011 and my surname changed, i came in to fourways branch subsequently to have it changed using my ID with my previous surname and my marriage certificate. Which was done. A year later today I came back to the branch to open 4 bank accounts for my kids which i had issues. As my ID Book still has my previous surname. What is totally unacceptable is that my account today a year later was changed back to my previous surname due to a person in the branch mistake at the time it was changed. My drivers lisense proves too that my surname has changed which the branch suddenly.now doesnt accept but did when my husband opened his account 2 years ago. I now have to spend money to get a new id and come back to the branch again just to have my surname changed!! I'm now inconvenienced because of a staff members mistake!!
