3RD CARD ISSUED WITH INCORRECT DETAILS

I've received my cheque card with surname being indicated as my first name. This is ridiculous!! Two previous cards was sent to the Pavilion branch and I never received any notification and the cards got destroyed. My 3rd request for a card resulted in my first name and surname being swopped around. I am NOT paying for another card and I am NOT going to the branch to request for another card. Sort this matter out immediately and issue me with the correct card and the correct details on it!!
