An Account i dont understand how it happened

I once phoned this company for a quotation in one of their loans and i spoke to the consultant and he did a quote to me, on that request he said for him to be able to draw that quotation i need to supply all my information which i did the bank account my id no etc, he then told me that should i accept the quote i will be required to pay R495 for credit check and he faxed me the quotation, and list of all the documents required to proceed with the loan and loan application forms, as i go through that quotation i realised that the loan was not what i thought it was and then decided not to continue with it, i did not complete the forms i just left everything as it was later i got a call from one of the consultant checking if Ive received the quotation and application forms and i explained to him that i will not be continuing with the quotation cause it is not what i want, and then it was okay, month end they debited R495 in my account then i was surprised i went to the bank and reversed the amount next month before my salary day i received the application form again by fax from them, i sent the consultant an email to cancel that quotation, but the debit order went again, and now they
