Account Paid up 2012 but still debited

My personal loan account got paid up last year 2012. However for some awkward reasons, sometime two months back I begin to get a debited for this paid up loan. I then personally took some effort to go to the branch and complain about this. The unacceptable thing is that, this month still, the debit ran. Even though they promised that the account will be closed!!! Meaning that my request was not met.

PLEASE CLOSE THIS ACCOUNT IMMEDIATELY.
