Loan account not closed and still being charged

When the personal loan was Settled, several calls and instructions to FNB were sent to close the account. This is several months ago. Last month for the first time in a while I was charged a fee (R147, 50) and refunded timeously. I assumed then that someone had used their brains to understand that if the refund was made surely the account should be closed/the customer contacted. Oh no, not the ever the innovative FNB (try innovating some real brains for your staff). Noticed again that this month the fee has gone off again. Please finally close this loan account, refund the fee and ensure (with proof to me) that the updated account status has been sent to the credit bureau (s) as my exposure (as per NCA) is still reflecting this account open. I have already transferred my other accounts to another bank and not taken additional accounts with FNB because their lack of after sales service/ability to not treat me as an imbecile. Please ensure this is done before the start of the new month or I will close too my FNB cheque account.
