Loan Application - Wrong Marital Status

I have done an online application for a loan and I confirmed this morning that all supporting documentation was received. This afternoon I received an SMS stating that I have to fax through my mariage certificate, how is this possible? I have never been married, I am single. My application forms and profiles everywhere state clearly that I am unmarried. Can someone please rectify this mistake on my application and process my application further. It seems someone has made a big mistake. Please advise me what caused the mistake urgently. Thanks
