Signed for something and never got it

Last year I upgrade my data contract with an voice contract (Blackberry 9780 Bold) and was help by Kim. I signed all the documentation even we're they asked if you would like to have the phone insured. Every month I pay my account like normal but on the 26 March 2013 my phone got. I reported the phone stollen in Sunnyside Vodacom shop just to be told that my phone was not insured. I come to the Brooklyn Vodacom shop and was told the same, but Craig the manager said that he will get the copy's of the signed documentation. Craig got it and was signed for the phone to be insured. Craig then said to me that he has spoken to Cellsure and they told him that it was my fold. But if Vodacom Brooklyn did there work nothing like this wouldn't happen. This morning 29 April 2013 I was told that I have to pay R1928.00 before to get a phone. Still don't don't see why I as the customer had to go through all this stress.
