On Upgrading of the phone

Some years back I entered into a contract with Vodacom, Early November I called to check when am I due for upgrade, I was told that I already upgraded. I was surprised because I never upgraded I was looking forward to do that. Another person did a and upgrade with the fake ID. The Photo on the ID is not mine, the name is not spelled correct (definitely not mine), date of ID issue is not the same as mine. I requested the contract she signed from the Game shop at Kempton Park (where she upgraded). I receive the last page of the contract, The other last page has a fake signature which is not mine, the name again is spelled incorrect.

I believe a sales person didn't pick up the mistakes of the names that are spelled incorrectly 2 times. (the ID and the last page of the contract) comparing the name on the system of existing contract. How can one forget her name?
I reported the case at Vodacom and submitted the affidavit from police station as they requested. The case was assigned to specific person at Vodacom. I have send the email to request the feedback. Up to now the person didn't say anything, and the debit is going on my account for 3 months for a phone dat I don't hav
