Still being charged on an expired contract

On 12 November 2013 I submitted a report explaining that I do not have any intentions to extend my contract after the expiry of my current contract. To my suprise end of December I was still charged for the full month. Moreover I also reported that I did not take up the second contract with cell number 0725782353 (Top Up 315) linked to my account. I did not conclude this contract and I have never even received a sim card for the said number. All I want is to keep my cellnumber 0726080728 as a pay as you go, I also request that you cancel the second contract and refund me all the amount taken from my account. I must also stress that this is the second time I get ill-treated by Vodacom when my contract expires.
